Tetany

Main cause of tetany is very low level of ionized calcium. This calcium deficiency might be because of several reasons.



The deficiency might result due to less presence of calcium in daily diet, or else Vitamin D deficiency. This deficiency might be caused due to low exposure to the sun. Vitamin D is essential for the body to absorb ionized calcium through the gastrointestinal tract.



Therefore vitamin D deficiency might result into low absorption of calcium and finally result into tetany.

It is not required that tetany might affect only the people with calcium deficiency, otherwise healthy people might experience it while going through certain situations like excessive vomiting due to stomach upsets. Tetany can be triggered also after hyperventilation.

Hyperventilation, during anxiety, and hard breathing removes too much carbon di oxide inside the body, thus resulting in higher levels of calcium proteinate and lower levels of free calcium thus increasing the risk of muscle excitability. Releasing of calcium from calcium proteinate is done by hydrogen inside the body, higher level of hydrogen means more calcium is freed and lesser the chances of tetany.



However, when the levels of hydrogen dip down, free calcium levels dips down and there are chances of muscle spasms or cramps.

Free calcium is important inside the body, it is only free calcium which can travel through capillary walls and determine the concentration of fluids inside tissues. This concentration helps controlling the electrical excitability of muscles. This controls almost all muscles, and even the heart muscles.





Tetany can be of high intensity or low intensity. Many people when faced with situations full of anxiety might get muscle spasms, but this might be limited only to tingling in toes and fingers as well as lips etc. However, tetany of high intensity can be fatal, if these muscle spasms include the laryngeal and pharyngeal muscles.



It is therefore, extremely necessary that low free calcium should be detected at the earliest.

Detection is generally done through tapping of facial nerve in front of the ear near the jaw. This test is known as ‘Chvostek’s Sign’, low calcium deposits through this method can be found out by twitching of the facial muscles as and reactions to tapping.
